Regeneración tisular guiada y microcirugía endodóntica en el manejo de un quiste radicular asociado a una lesión through-and-through: Reporte de caso

Abstract

Non-surgical endodontic treatment has a high success rate, although biological and technical factors can influence the outcomes. Periapical lesions, such as periapical cysts, are common causes of failure. These lesions can result from intraradicular or extraradicular infections, and their treatment may require apical surgery. In this case, a patient with a periapical cyst was treated through a microsurgical procedure assisted by a microscope, followed by guided tissue regeneration (GTR). The surgery included apicoectomy and retrofilling with bioceramic materials, as well as the placement of lyophilized bovine bone and a collagen membrane. Clinical and radiographic results were favorable, with a complication-free recovery. Histopathological analysis confirmed the diagnosis of a periapical cyst. Endodontic microsurgery showed a high success rate in this type of lesion, and the use of GTR improved bone regeneration. One year after the procedure, the patient remained asymptomatic and showed good progress. This case highlights the effectiveness of endodontic microsurgery and GTR in the treatment of complex lesions, such as radicular cysts.
