The present case involves a 17-year-old female patient with fixed orthodontic appliances who presented clinically with an increased mesiodistal crown diameter of tooth 11, along with a definitive diagnosis of symptomatic irreversible pulpitis and symptomatic apical periodontitis. Periapical radiography and cone-beam computed tomography revealed anatomical characteristics of Dens invaginatus type IIIb, as classified by Oehlers. Treatment included the use of magnification, a rotary system, disinfection with mechanical activation, apical sealing with hydraulic cement, and thermoplastic obturation.
